Some apps will not allow for reliable grouping (Adobe Acrobat reader for example) and have to be excluded.  Often, the reason for this is that the application has a custom title bar (does not conform to a Windows standard title bar).  If other apps are grouping correctly, but one is not, this is likely the cause - it has a custom title bar.  This may be something worth further investigation in query to the developer by users.
